Podmiotowe uwarunkowania adaptacji pacjentów do życia z chorobą psychosomatyczną układu pokarmowego
Abstract
Problem badawczy pracy brzmi: jakie są podmiotowe uwarunkowania adaptacji pacjentów do życia z chorobą psychosomatyczną układu pokarmowego. Przebadano 144 osoby w wieku 18-60 lat (M= 33,43; SD= 10,23) z psychosomatycznymi chorobami układu pokarmowego. Wykorzystano: wywiad częściowo ustrukturyzowany, Kwestionariusz Oceny Stresu, Skalę Odczuwanego Stresu, Wielowymiarowy Inwentarz do Pomiaru Radzenia Sobie ze Stresem, Skalę Akceptacji Choroby, Wielowymiarowy Kwestionariusz do Badania Obrazu Ciała. Wykazano: 1) związki między nasileniem stresu i oceną choroby jako stresora a wymiarami adaptacji do życia z chorobą i stylami radzenia sobie ze stresem; 2) mediacyjną rolę: stylu skoncentrowanego na problemie w relacji między oceną choroby jako wyzwanie i zorientowaniem na choroby; poszukiwania wsparcia i stylu skoncentrowanego na emocjach w relacji między oceną stresora jako wyzwanie_aktywność i zorientowaniem na choroby; stylu skoncentrowanego na zachowaniach unikowych w relacji między oceną stresora jako wyzwanie i oceną zdrowia; stylu skoncentrowanego na problemie w relacji między oceną stresora jako wyzwanie_aktywność i oceną zdrowia. Analizy wskazują na istotną rolę oceny choroby przewlekłej jako stresora dla procesu adaptacji do życia z chorobą psychosomatyczną układu pokarmowego.
The research problem of the study is: what are the subjective determinants of patients adaptation to life with a psychosomatic gastrointestinal disorder. A total of 144 patients aged 18 - 60 years (M= 33.43; SD= 10.23) with psychosomatic gastrointestinal disorder were studied. The following measures were used: a semi-structured interview, Stress Assessment Questionnaire, Perceived Stress Scale, Multidimensional Coping Inventory, the Illness Acceptance Scale, and the Multidimensional Body Image Questionnaire. It showed: 1) associations between stress severity and assessment of illness as a stressor and dimensions of adaptation to living with disorder and coping styles; 2) mediating role of: problem-focused style in the relationship between illness orientation as challenge and illness orientation; support-seeking and emotion-focused style in the relationship between stressor appraisal as challenge_activity and illness orientation; avoidance behaviour-focused style in the relationship between stressor appraisal as challenge and health evaluation; problem-focused style in the relationship between stressor appraisal as challenge_activity and health evaluation. The analyses indicate the important role of chronic disease assessment as a stressor for the process of adaptation to living with a psychosomatic disorder of the gastrointestinal system.
